TRANSITION CARE MANAGEMENT (TCM)
Patients discharged from the hospital need a strong, reliable support system to aid and progress in the recovery process. Our professional telepsychiatrists provide guidance and clarity in the transition stage for outpatients. Within seven days of discharge, a licensed counselor will conduct a Telemedicine Assessment. We do this from the comfort of the patient's home. Additional assistance is critical at this stage to prevent relapse. Change is tough on everyone, but we're here to help the transition process go as smoothly as possible. We work with Aetna Better Health, AmeriHealth Caritas of Louisiana, Medicare, and Medicaid patients to ensure that everyone can get the professional help that they deserve.

This allows our psychiatrists to get more vulnerable and honest responses. We now offer this service to all patients residing in the state of Louisiana.
